The current Tamil Nadu Proforma-I specifically instructs the applicant to sign in the old name in the prescribed place. Applicants should therefore follow the signature instructions appearing on the current Government form.

12. Is there a special requirement for an applicant aged 60 years or above?

Yes. The current Proforma contains a Life Certificate requirement for applicants aged 60 years or above. The certificate is to be obtained from a Government Medical Practitioner and enclosed according to the current Government instructions.

13. How many Gazette copies are provided after publication?

The current Government instructions provide for five copies of the Gazette containing the individual's name-change publication. Additional copies can be obtained according to the applicable Government procedure and payment requirements.

14. What should an applicant do if the published Tamil Nadu Gazette contains a mistake?

The Gazette should be checked immediately after publication. Under the current Proforma-I instructions, a discrepancy in the printed Gazette should be brought to the department for rectification within six months. Applicants should therefore promptly verify the old name, new name, initials, date of birth, address and other published particulars rather than discovering the mistake much later.
